30-Day Transition Support Sprint

The 30-Day Transition Support Sprint is designed for parents and families who are ready to move from “we know something needs to change” to “we have a plan we can actually start using.”

This short support package helps families identify one important transition priority, build a simple 30-day plan, review what is working, and adjust next steps before things become overwhelming.

This service is especially helpful for families supporting a teen or young adult who may need support with independence, responsibility, executive functioning, routines, college readiness, work readiness, or life after high school.

The goal is not to fix everything at once. The goal is to create one realistic focus area, build momentum, and help the family understand what kind of support may be needed next.

What is included:

• 3 sessions over 30 days
• Session 1: Build the 30-day transition action plan
• Session 2: Review progress, barriers, and needed adjustments
• Session 3: Clarify next steps and ongoing support needs
• Identification of one priority transition focus area
• Practical action steps for the parent and young adult
• Brief between-session check-in support
